Home to Stay – Home Modifications and Community Support Toolkit

The Home to Stay Program, Stella’s Circle, identified [the need] and supported seniors with complex mental health needs to age safely in their community. A Home Modifications and Community Support Toolkit was developed to share the learnings from the project and provide guidance to organizations and individuals facing similar challenges.
